Profiles with no name or picture

This may seem a silly question, but I've been around a long time, yet more recently I've noticed there are any number of profiles posting comments on other profiles, yet the poster has no pic (which isn't a real issue), but there is also no user name appearing in the comment.
Is this just part of the new upgrades, or are these somehow old accounts that have been deleted, but their comments remain? Honestly, I'm just curious.

If it says guest where the profile name should appear then it was an account that was deleted.

They look like this:
I'm guessing the ones from very recently may be under review, though I didn't think you could comment until it was verified, or fully active.

Deleted member. Profile comments don't always seem to delete with the member.
